XXII Conference AIIC / 4. Evaluation of health technologies at a turning point

In the opinion of Lorenzo leogrande, President of the CNA National Conference in Riccione, “we are living in an optimal institutional and regulatory moment to relaunch the HTA in close relation to recruitment and other professions.

The evaluation of technologies to be acquired in health realities, the HTA (Health Technology Assessment) “is a key topic at the 2022 Conference for two reasons. The first refers to our competence and profession: we, in fact, take care of HTA every day in the activity that we develop. The second reason is linked to the really important institutional moment we are living in “, are the words of Lorenzo Leogrande, President of the XXII National Conference of the CNA, Italian Association of Clinical Engineers, ongoing in Riccione. This is a real “relaunch” of the importance of hypertension in the Italian health system, operated in the AIIC event, which highlights known issues and stimulates new scenarios: “Through the Delegate Law 53/2021 -remembers Leogrande- the national legislation for the two regulations 745/2017 and 746/2017 relating to health products and in vitro devices, in which there will also be a part related to the strengthening of hypertension, especially with regard to That is why we wanted to address the issue in a direct confrontation with the purchasing centers, which deal with the purchase process on a daily basis ”.

Today, the link between hypertension and purchasing is substantially absent because “purchasing centers – continues the president of the AIIC Conference – collect the needs of healthcare companies, group demand and, in innovation tenders, adopt allocation mechanisms. direct. Instead, we expect the spread of a culture of hypertension that is applied regardless of methodologies and regulatory obligations. Then we have to work so that the purchasing processes take into account, whenever possible, the valuations made. It is clear that this connection must be created from a common language. “

Since 2017, with the implementation of the HTA control room that clearly and precisely set out the process of purchasing new technologies, the model has had difficulty translating into practice. In fact, the functionality of a rational and structured model, sanctioned by a State-Regions agreement, of a control room – formed by the Ministry of Health, National Agencies (Agenas and Aifa) and representatives of regions – for production of HTA. “Despite the halt of the pandemic years, the regions have used elements of the process and therefore a working group has been set up whose mission is to try to make this relationship more efficient,” he said. Americo Cicchetti, director of Altems, the Baccalaureate of Economics and Management of Health Systems. A relationship that, however, needs to be stabilized, updated and relaunched.

As highlighted by the experts involved in the debate within the CNA event, today it is necessary to update the State-Regions Agreement of 2017 that defined the national HTA program and, subsequently, to provide for a periodic update to make -authorized and representative for all subjects involved. In addition, “resources and responsibility are needed – was the message proposed by Cicchetti – among the missing elements in this moving scenario, without a doubt, one refers to the resources, because it has always been thought to develop the Evaluation of Health Technologies without investments and this obviously makes the whole system slow if not immobile “.

In conclusion, Lorenzo Leogrande emphasized that the CNA not only “observes the entire HTA system from the inside, but also aims to be its engine. We, as professionals and together with other professionals, are fully aware of the current advantages and criticalities and will be the protagonists of proposals and suggestions to national and regional decision-makers, so that we do not miss the opportunity offered by the PNRR and recent regulations. Healthcare Technology Assessment can find an Italian way to assess healthcare technologies that can deliver real value, ensuring sustainability and proper management. “
